"Graphics Shop" (for lack of a better name)

Right, I know I'm not amazing, but I love making graphics and lately I've had few requests from anyone including myself. Examples of my work, as well as the information I need, is below. I make signatures, avatars, or larger-size blends. Avatars and signatures can match if you so choose (If you would like me to make a matching "set" please make a note under "other")

The "Form"
I need this information to create any graphics. If you do not want one of the form fields to apply (for example, no name) just ignore the field. If a field is left blank I will assume you don't want it to be on the signature. I will find all photographs myself unless you really want me to use one (if so, put the URL under "other"). I don't purely make cat pictures, though for this site I expect most requests will be of cats ^^ You need to fill out a form for each seperate graphic that you would like me to make

Character's Name: (self explanatory)
Character's Appearance: brief description of character
Colors you would like me to use: list a few colors you would like me to use in your piece
Background: (description, please. I will provide pictures myself)
Other effects on picture: changed colors, light bursts, etc.
Text: Do you want text? If so, what should it say (other than your cat's name)?
Graphic type: Avatar, signature, or larger-sized blend
Other:

NOTE: Under no condition should you use or display these pictures without my consent, nor should you ever post them without giving credit to me. Some of these are requests done for other people and using them as your own is unacceptable.

Background: Hmm...I don't really care. Maybe keep the background the same, but... I'm not sure. Any way that you can hide the flowerpot? =3

Other effects on picture:Find a cool brush effect that will fit the picture. I trust you to find something nice. ^^

Character's Name: Melodypaw/Melodysong (Haven't made her yet

Character's Appearance: Melodypaw is an all white she-cat except for her face. Her face is a mix between brown and carmel colors. She has pretty blue eyes.
